Survey Design

The Emotional Health History and Well-Being survey asks about a participant’s emotional and mental health as it relates to topics such as anxiety, depression, stress, and beyond. The survey includes 110 items (46 main items and 64 related follow-ups) from the following sources: the Generalized Anxiety Disorder scale (GAD-7), the World Health Organization World Mental Health Composite International Diagnostic Interview (CIDI), the Patient Health Questionnaire depression scale (PHQ-9), the UK Biobank Mental Health Questionnaire, the Self-Injurious Thoughts and Behaviors Interview (SITBI), the Behavioral Risk Factor Surveillance System (BRFSS), the PTSD Checklist (PLC-C), and items developed specifically for use within the All of Us Research Program.

Scoring Considerations for the Behavioral Health and Personality Survey

The Emotional Health History and Well-Being survey is composed of items from various sources so can not be scored as a whole. The survey does, however, include items from several scales that can be scored on their own. These include the Generalized Anxiety Disorder scale (GAD-7), the Patient Health Questionnaire depression scale (PHQ-9), and the Behavioral Risk Factor Surveillance System (BRFSS). Sample Eligibility

The survey is available to all consented participants who have completed the first three baseline surveys: The Basics, Overall Health, and Lifestyle. For more information about survey samples and releases, please see Introduction to All of Us Survey Collection and Data Transformation Methods.

All survey codebook variables (e.g., source codes) are deposited into the OMOP Common Data Model’s observation table and stored as concepts in an All of Us specific “PPI” vocabulary.
