Using the Temporal Feature within the Cohort Builder

Note: We recommend reading “Selecting participants using the Cohort Builder” before reading the following article as a basic understanding of the Cohort Builder is helpful to using the temporal feature.

To create a cohort with temporal criteria, you can use the temporal feature in the point-and-click tool, the Cohort Builder.

Temporal criteria can be inclusion or exclusion criteria and considers clinical events that happened before, after, within a certain time range, or during the same time range to another clinical event. Before defining the temporal criteria, you also want to consider the timing of when the clinical event appears in the data, including any mention, first mention, and last mention.

Note: When using the temporal feature, you should read from top to bottom. It is important to keep in mind the structure of the temporal feature before entering all criteria. Even though the temporal selection is added above the temporal key, there is a continuous line that shows a division or delimitation between the temporal criteria and any prior or additional selection.

To create your first criteria and toggle the temporal feature

  1. Click “white plus sign on top of a blue circle
” to the right of “Cohorts.”

    The workspace’s data tab includes the Cohort Builder and the Dataset Builder. The Cohort Builder is displayed on the left inside of the screen in a white card with the title Cohorts. A cohort is a group of participants based on specific criteria. Think of your cohort as a subsection of all of the All of Us participants. The white plus sign on top of a blue circle is to the right of the “Cohorts” title. After you click the white plus sign on top of a blue circle, the Build Cohort Criteria page appears with include participants, group 1, and add criteria.

  2. Click “Add Criteria” for a drop-down menu of program data, domains, and concepts to select from for your inclusion criteria.

    After clicking add criteria button, a dropdown appears. You can search or browse through the program data, domains, and concepts available in the Researcher Workbench. This includes demographics, surveys, physical measurements, conditions, procedures, etc.

  3. Search or browse the program data, domains, and concepts.



  4. Select a program data, domain, or concept that you want to add as inclusion criteria for your cohort.

    To select the program data, domains, and concepts, click on the text of interest. Depending on the data, domain, or concept you select, a screen will appear related to that specific data, domain, or concept. For example, clicking on physical measurements will bring up a screen that includes additional selections related to blood pressure, heart rate, height, weight, and BMI.

  5. Define the inclusion criterion based on your research project.



  6. Click “Finish & Review.”

    The Finish & review button is at the bottom right of the screen and is grayed out until you finish and select your first inclusion criteria. The button will turn dark blue. After you click the Finish & review button, a pop up appears on the right hand side. The pop up provides you a summary of the criteria you selected during the prior steps. You can remove the criteria by selecting the red circle and x to the left of the criteria listed.

  7. Click “Save Criteria.”

    After you click save criteria, the screen will return to the Build Cohort Criteria page. Next you will add the temporal feature.

  8. Toggle the slider next to “Temporal.”

    After you toggle the temporal feature on in the Cohort Builder, additional fields will appear, including the timing of when the first criteria (the criteria you just selected) appears in the data. The timing of when the first criteria dropdown appears above the criteria you just selected.

  9. Select the timing of when the first criteria appears in the data: any mention, first mention, and last mention.
    • Any mention: Represents any time the specific code appears in the data.
    • First mention: Represents the first time the specific code appears in the data.
    • Last mention: Represents the last time the specific code appears in the data.

To create your second criteria and apply the temporal criteria

  1. Click “Add Criteria” right above the temporal toggle function.

    After clicking the add criteria button, a dropdown appears. You can search or browse through the program data, domains, and concepts available in the Researcher Workbench. This includes demographics, surveys, physical measurements, conditions, procedures, etc.

  2. Search or browse the program data, domains, and concepts.



  3. Select a program data, domain, or concept that you want to add as inclusion criteria for your cohort.

    To select the program data, domains, and concepts, click on the text of interest. Depending on the data, domain, or concept you select, a screen will appear related to that specific data, domain, or concept. For example, clicking on physical measurements will bring up a screen that includes additional selections related to blood pressure, heart rate, height, weight, and BMI.

  4. Define the inclusion criterion based on your research project.



  5. Click “Finish & Review.”

    The Finish & review button is at the bottom right of the screen and is grayed out until you finish and select your second inclusion criteria. The button will turn dark blue. After you click the Finish & review button, a pop up appears on the right hand side. The pop up provides you a summary of the criteria you selected during the prior steps. You can remove the criteria by selecting the red circle and x to the left of the criteria listed.

  6. Click “Save Criteria.”

    After you click save criteria, the screen will return to the Build Cohort Criteria page. Next you will define the relationship between the second criteria and the first criteria. The relationship between the second criteria and the first criteria dropdown appears above the second criteria you just selected.

  7. Select the relationship of the second criteria in relationship to the first criteria: during the same encounter as, X or more days after, X or more days before, or on or within X days.

To apply during the same encounter as

  1. Select “During the same encounter as.”

    During same encounter as is the first option displayed on the menu.

  2. Review your criteria and temporal function.

    On the right panel, you can see a running total, or “Total Count” of participants that will be included in your research project’s cohort based on your inclusion criteria with the temporal feature. The refresh button is on the top right of the Total Counts section panel on the right of your Builder Cohort Criteria screen. In the Total Counts panel, you can see your cohort by race and race and age at CDR. On the top right of the Cohort Builder, there is a blue create cohort button.

  3. Click “Create Cohort.”

To apply X or more days after

  1. Select “X or more days after.”

    X or more days after is the second option displayed on the menu. A text field box will appear to the right of the drop down you selected X or more days after in.

  2. Type the number of days to the right of the “X or more days after.”



  3. Review your criteria and temporal function.

    On the right panel, you can see a running total, or “Total Count” of participants that will be included in your research project’s cohort based on your inclusion criteria with the temporal feature. The refresh button is on the top right of the Total Counts section panel on the right of your Builder Cohort Criteria screen. In the Total Counts panel, you can see your cohort by race and race and age at CDR. On the top right of the Cohort Builder, there is a blue create cohort button.

  4. Click “Create Cohort.”

To apply X or more days before

  1. Select “X or more days before.”

    X or more days before is the third option displayed on the menu. A text field box will appear to the right of the drop down you selected X or more days before in.

  2. Type the number of days to the right of the “X or more days before.”



  3. Review your criteria and temporal function.

    On the right panel, you can see a running total, or “Total Count” of participants that will be included in your research project’s cohort based on your inclusion criteria with the temporal feature. To apply on or within X days

  1. Select “On or within X days.”

    On or within X days of is the fourth option displayed on the menu. A text field box will appear to the right of the drop down you selected on or within X days of in.

  2. Type the number of days to the right of the “On or within X days.”



  3. Review your criteria and temporal function.

    On the right panel, you can see a running total, or “Total Count” of participants that will be included in your research project’s cohort based on your inclusion criteria with the temporal feature. The refresh button is on the top right of the Total Counts section panel on the right of your Builder Cohort Criteria screen. In the Total Counts panel, you can see your cohort by race and race and age at CDR. On the top right of the Cohort Builder, there is a blue create cohort button.

  4. Click “Create Cohort.”

The cohort you created can now be used to create a dataset using the Dataset Builder.

Note: The temporal feature sets the query parameters to identify person IDs based on the inclusion and exclusion criteria you select in the Cohort Builder. The feature does not extract the data. Additional data wrangling and cleaning of the final dataset may be needed.
